Good Morning,
Quick question: Are there any guidelines about when to increase (or otherwise adjust) the smoothness difficulty levels? Is there a certain percentage or something? Say, if you are consistently averaging 75%, do you go to the next level, or what?

Its not so clear cut. This depends more on what motivates your continued training, and provides enjoyment, and if you like, challenge.
Some people may prefer to stay on standard. I would say if you are on 100% the whole time, or notice that when you are not breathing well it still stays smooth, its time to go up, as that isn't useful feedback. Otherwise it depends on the person.
As a guess, it sounds like you should go up to the next difficulty level.
